It would be a good idea to read some books, or at least some Forum threads, on the subject first and only then ask more specific questions, for example "Do you have any advice on formulating/wording a statement of intent correctly?" because there've been all sorts of debates already whether to include "It is my wish..." as in Carroll's book or not, about using the future or the present tense, specificity v. generality, whether to include avenues of manifestation, etc.
